The Application for the 2012 Kick, Lead and Dream Camp can be easily downloaded by clicking on the icons below (English and Spanish versions). After downloading, please print it and fill it out. You have multiple ways of submitting it:
- hand it to your school office (if they are accepting them) or the school resource officer
- drop it off at the Mountain View Police Department or the Sunnyvale Police Department
- mail to Kick, Lead and Dream, PO Box 3907, Los Altos, CA 94024
